For consideration of a grant with the Cook Family Foundation an application must be submitted. You can access our application via Google Forms Here.

The Cook Family Foundation Board meets quarterly and reviews grant applications at each board meeting. The current deadline is April 1st, 2025 for grants to be reviewed at the April 2025 Board Meeting. Rolling applications are acceptable and will be reviewed pending the time they are submitted relative to the next Board Meeting.
